Community Health Needs Assessment Survey

Dear Neighbors,

The Community Health Department at MedStar Health, which manages Good Samaritan and Union Memorial Hospitals, oversees and coordinates the Community Health Needs Assessment process, required to be completed every 3 years as a part of the Affordable Care Act, to improve health outcomes throughout the system’s service areas. Please complete the online or attached survey to communicate your community health needs and inform MedStar on the community health priorities they should pursue.

  • Paper Copy: MUMH Flyer and Paper Copy of surveys can be provided in English, Spanish, Korean, and Amharic

  • If you want MedStar to mail paper copies to your community or organization, reach out to Deborah Bena – Deborah.Bena@medstar.net or 443-444-4793. She can also coordinate pick up of completed copies from your location/organization.

  • Deadline for completion – October 23, 2020
